I also certify that I have received and reviewed a copy of the Student policy packet, which contains further details on DAAYO's policies and expected student behavior.Waivers General Waiver In consideration for my child(ren)’s in-person participation in Dallas Asian American Youth Orchestra’s (hereafter referred to as “DAAYO”) programs, services and facilities for the 2025-2026 season (August 24, 2025 to May 17, 2026) I, the undersigned, as the parent or legal guardian acknowledge and agree to all the policies outlined in the DAAYO Policies and Student Packet 2025-2026, which I have received either via email or downloaded from google classroom. The undersigned and their student acknowledges and agrees to adhere to all the policies outlined in the packet, which include attendance, health screens, media use, and other misc. orchestra operational policies. DAAYO reserves the right to make changes to these policies as deemed necessary by DAAYO's administration and/or changing circumstances. DAAYO reserves the right to remove any student from our programs without refund due to inappropriate behavior, attendance issues, or other issues as determined by DAAYO’s administration. By signing this agreement, the undersigned acknowledges and agrees to the above. Additionally, the undersigned also agrees to voluntarily assume the risk for themselves and their child(ren) of participation in DAAYO’s educational and performance programs. The undersigned understands that DAAYO and its staff will take reasonable precautions to ensure the safety and well-being of participants as well as the security / safety of personal property, but that DAAYO cannot eliminate all risks. The undersigned hereby releases, waives, discharges, and holds harmless DAAYO, its officers, employees, agents, and volunteers from any and all claims, liabilities, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by the undersigned’s child or to any property belonging to my child while participating in DAAYO. The undersigned acknowledges that they have read this waiver and release of liability and fully understand its terms, and that they are signing it freely and voluntarily without any inducement. Photography/Video/Media Waiver The undersigned acknowledges that Dallas Asian American Youth Orchestra (DAAYO) may take pictures, make audio or video recordings or capture digital images of the undersigned while the undersigned appears at DAAYO events or rehearsals. The undersigned hereby consents to being photographed or recorded. The undersigned further agrees that the copyright in any pictures, images, videos or audio recordings shall belong to DAAYO and may be used and re-used by DAAYO, either separately, or together with other photographs, images or audio or video recordings. The undersigned releases and discharges DAAYO from any claims arising from or related to the use of any photographs, digital images, audio or video recordings of the undersigned. If the undersigned is younger than eighteen years of age, then this Consent and Release shall be signed by the parent or other legal guardian of the undersigned.
